Most people know whether they’re type A, B, AB, or O, but fewer understand the immunology behind that label — or how accurate the "universal blood type" claim really is.

What determines your blood type

The ABO system classifies blood by which antigens sit on the surface of red blood cells: only the A antigen makes you type A, only the B antigen makes you type B, both make you AB, and neither makes you O.

Your plasma carries antibodies to match

Your plasma naturally contains antibodies against whichever antigen your own red cells lack — someone with type A blood, for example, typically carries anti-B antibodies. This is exactly why blood types can’t be mixed freely during a transfusion.

The Rh factor is the "positive" or "negative" part

On top of the ABO grouping, whether red cells carry the RhD antigen determines the plus or minus sign after your letter — "O positive," for instance, means type O blood that is also RhD-positive.
